Quick answer

FEP Blue - the Blue Cross and Blue Shield Federal Employee Program - covers substance use disorder treatment, including detoxification, outpatient counseling, residential treatment centers, and inpatient rehab. The one rule that catches people off guard: planned hospital stays require precertification, and skipping it costs a $500 penalty.

FEP Blue is the Blue Cross and Blue Shield Federal Employee Program - the plan most federal employees and retirees enroll in through the FEHB Program. Because FEP Blue prints its benefits in a public OPM brochure, you can know exactly what it covers for rehab before you call anyone. The short version: substance use disorder treatment is covered at every level of care, but planned hospital stays need precertification first, and skipping it triggers a $500 penalty.

Does FEP Blue cover rehab and detox?

It depends on the level of care and the facility. Outpatient counseling, medication management, partial hospitalization, and intensive outpatient treatment are covered broadly under Section 5(e) of the 2026 brochure. Inpatient rehab in a hospital or treatment facility - including room and board and ancillary charges for the rehabilitative treatment of alcoholism or substance use disorder - is covered, but requires precertification. Residential treatment centers are covered separately and also require precertification. Sober living homes, halfway houses, and group homes are not covered, so ask a facility exactly how it is licensed before you commit.

What FEP Blue is, and why federal rules shape the benefit

FEP Blue is not a separate insurance company. It is the Blue Cross and Blue Shield Service Benefit Plan, the Federal Employee Program (FEP) that most federal employees and retirees enroll in through the FEHB Program - a fee-for-service plan with a preferred provider organization administered by the Blue Cross and Blue Shield Association under contract (CS 1039) with the U.S. Office of Personnel Management. The plan comes in three options - FEP Blue Standard, FEP Blue Basic, and FEP Blue Focus - with the Standard and Basic Options covered together in brochure RI 71-005 and Focus covered by its own brochure (RI 71-017).

Two federal rules shape what you get. First, under the Mental Health Parity and Addiction Equity Act, OPM requires FEHB carriers to cover mental health and substance use disorder treatment on terms comparable to medical care, including the 2021 amendments on non-quantitative treatment limitations. Second, the brochure itself confirms this happens in practice: it states that every year FEP Blue conducts an analysis of the financial requirements and treatment limitations applying to its mental health and substance use disorder benefits in compliance with the Act, and that under Standard Option, cost-sharing and limitations for in-network and out-of-network mental health and substance use disorder benefits are no greater than for similar benefits for other illnesses and conditions.

What the 2026 brochure covers for substance use disorder

Section 5(e), Mental Health and Substance Use Disorder Benefits, covers professional services by licensed professional mental health and substance use disorder practitioners acting within the scope of their license:

  • Individual psychotherapy and group psychotherapy
  • Pharmacologic (medication) management and psychological testing
  • Office visits, clinic visits, and home visits
  • Phone consultations and online medical evaluation and management services (telemedicine)
  • Telehealth professional services for behavioral health and substance use disorder counseling

Facility-level care is covered at the levels shown in the brochure's own cost table:

  • Inpatient services provided and billed by a hospital or other covered facility, with room and board and ancillary charges for the rehabilitative treatment of alcoholism or substance use disorder
  • Residential treatment center inpatient care for medical, mental health, and/or substance use disorder treatment, when the facility is licensed as an RTC
  • Outpatient services provided and billed by a covered facility, including partial hospitalization and intensive outpatient treatment
  • Outpatient services provided and billed by residential treatment centers, at the outpatient levels shown

If you prefer care from home, telehealth counseling with a Preferred telehealth provider costs nothing under both options, and the plan's teleconsultation line (855-636-1579) covers counseling for behavioral health and substance use disorder as well.

What you pay, by option

Under Standard Option, the calendar year deductible is $350 per person ($700 per Self Plus One or Self and Family enrollment); Basic Option has no calendar year deductible. As of the 2026 plan year:

  • Professional MH/SUD office visits: Standard Option, Preferred $30 copayment per visit (no deductible); Basic Option, Preferred $35 copayment per visit.
  • Inpatient hospital or other covered facility: Standard Option, Preferred facilities $350 per admission copayment for unlimited days (no deductible); Member facilities $450 plus 35% of the Plan allowance. Basic Option, Preferred facilities $425 per day copayment up to $2,975 per admission; Member and Non-member facilities - you pay all charges.
  • Residential treatment center (inpatient): Standard Option, Preferred facilities $350 per admission copayment for unlimited days (no deductible); Member facilities $450 plus 35% of the Plan allowance; Non-member facilities 35% of the Plan allowance. Basic Option, Preferred facilities $425 per day copayment up to $2,975 per admission.
  • Outpatient services at a covered facility (including partial hospitalization and intensive outpatient treatment): Standard Option, Preferred 15% of the Plan allowance (deductible applies); Basic Option, Preferred $35 copayment per day per facility.
  • Telehealth counseling, Preferred telehealth provider: Nothing (no deductible) under both options.

Every one of these figures comes straight from the plan's 2026 brochure as of October 2026; confirm your exact cost share with Member Services, since your situation may include Medicare coordination (the brochure waives telehealth copayments for members with Medicare Part B primary).

The precertification rule that catches people

The brochure puts it in capital letters: you must get precertification for hospital stays, and failure to do so will result in a $500 penalty. Residential treatment centers require precertification prior to admission as well. Before any planned admission to rehab or detox, call the customer service phone number on the back of your ID card and let the plan's care coordination staff confirm the facility and level of care. Emergency admissions follow different rules, but a planned rehab stay is the case where people most often lose $500 they did not need to lose.

What is not covered

The brochure is specific about the edges of the benefit. RTC benefits are not available for facilities licensed as a skilled nursing facility, group home, halfway house, or similar type facility. Inpatient and outpatient not-covered services include services performed or billed by schools, halfway houses, group homes or members of their staffs, psychoanalysis or psychotherapy credited toward earning a degree, and testing for and treatment of learning disabilities. Certain ancillary services sometimes bundled with rehab - recreational therapy, educational classes, biofeedback, outdoor residential programs, and equine therapy during an approved stay - are also excluded, so ask how a program is billed rather than assuming everything it offers is covered.

How to use FEP Blue for treatment, step by step

  1. In a medical emergency, call 911. For a mental health or substance use crisis, call or text 988.
  2. Confirm your coverage before you enroll in a program. Call Member Services at the number on the back of your ID card, or use the National Doctor & Hospital Finder to locate Preferred providers and facilities.
  3. Get precertification for any planned inpatient or RTC admission. Skipping it costs $500 under the plan's rules.
  4. Ask any facility whether it is licensed as a residential treatment center. If the answer is "residential program" without a state license, inpatient coverage may not apply.
  5. Compare levels of care. If outpatient counseling with a $30 or $35 copay can meet your needs, it may avoid the inpatient cost-sharing entirely.

Where to call for help

For help finding treatment and checking your FEP Blue benefits, call (800) 653-9376. The call is free to you, the referral service is paid for by treatment providers, and there is no judgment waiting on the line. SAMHSA's National Helpline (1-800-662-4357) is a free, confidential alternative open around the clock.

Questions about Fep Blue Coverage

Does FEP Blue cover rehab and detox?

Yes. FEP Blue's 2026 Service Benefit Plan brochure lists inpatient treatment of alcoholism or substance use disorder - including room and board and ancillary charges - among covered benefits, and Section 5(e) covers outpatient counseling, partial hospitalization, and intensive outpatient treatment by licensed mental health and substance use disorder practitioners. Medication management and psychological testing are covered too.

What is the $500 precertification penalty?

You must get precertification for inpatient hospital stays. FEP Blue's brochure states that failure to precertify will result in a $500 penalty, so call the customer service number on the back of your ID card before a planned admission. Residential treatment centers also require precertification prior to admission.

How much will I pay for substance use disorder treatment?

Under Standard Option, mental health and substance use disorder professional visits cost a $30 copay per visit with Preferred providers, and a Preferred facility charges a $350 copayment per admission for unlimited days. Under Basic Option, professional visits cost a $35 copay with Preferred providers and a Preferred facility charges $425 per day, up to $2,975 per admission. Both options say your cost-sharing responsibilities are no greater than for other illnesses or conditions.

Is outpatient rehab covered by FEP Blue?

Yes. FEP Blue covers outpatient services provided and billed by a covered facility, including individual psychotherapy, group psychotherapy, medication management, partial hospitalization, and intensive outpatient treatment. It also covers outpatient mental health and substance use disorder services provided and billed by residential treatment centers.

Are sober living homes covered?

No. FEP Blue's brochure excludes services provided or billed by schools, halfway houses, group homes, or their staffs, and residential treatment center benefits are not available for facilities licensed as a skilled nursing facility, group home, halfway house, or similar type facility. A facility must be licensed as a residential treatment center for inpatient coverage.

Does FEP Blue cover telehealth counseling for addiction?

Yes. The brochure covers telehealth professional services for behavioral health and substance use disorder counseling, and a Preferred telehealth provider costs nothing (no deductible) under both Standard and Basic Option.
